This form covers April 27 - July 19 practices and needs to be completed BEFORE MARCH 28th. Athletes' reported availability will be considered when assigning routines, so please be as through as possible when completing the form.

Please note that the following dates are mandatory: May 30th (intro/ finale learning), July 6th - July 19th (swimmers not participating in Champs July 6th - July 11th). Intermediate and USA swimmers in the Floating Routine must attend the practice on May 28th.

Swimmer attendance on this form will be used for routine assignments and practice scheduling. If you are unsure whether your athlete will be available on a specific date, please mark "NO." We may be unable to reschedule routine learning dates or routine practices if your daughter marks "YES" on the form, but is subsequently unable to attend. 

You will receive a confirmation email with all of the info you entered on the form once you complete it.  Please keep this for your records to keep track of which days you indicated your daughter would not be at practice.  If you need to make changes after the form is submitted, you must email those changes to Jennifer at info@aquastarz.org.


Please list your available days to coach/lifeguard on this form.  Your scheduled shifts will be listed in Tsheets once shifts are assigned.
